On the Place Order page, select the desired repairs from the appropriate drop down menu and "add to cart". When you are finished with all of your selections "checkout".
When you checkout you have the option to pay with PayPal or to pay with a credit card. Fill in the required information and continue.
Once your order has been completed, you should receive a confirmation email from PayPal. You should also be redirected to our customer information form. Fill out the form, print page, and include with your orthotics (if you miss the form, you can find a link on the Place Order page).
Finally, package your orthotics along with the customer information form in an appropriate shipping container such as a small box or reinforced envelope (make sure to tape the flap on self sealing packages so they don't open in transit). Ship your orthotics using the carrier of your choice (we strongly recommend you use a service that includes tracking).
